Complete the information requested for each box. Your request must be submitted NO LATER THAN Wednesday of the third week of classes for consideration. Due to limited staff and student accompanists, NOT ALL REQUESTS CAN BE FILLED. Assignments will be finalized and posted under "student resources/accompanist requests" at www.music.gsu.edu at the beginning of the fourth week of classes. No additional assignments will be made following this date. IF YOU NEED AN ACCOMPANIST FOR MORE THAN ONE LARGE ASSIGNMENT, FILL OUT MORE THAN ONE REQUEST.

A copy of each selection to be accompanied must be submitted to the assigned pianist NO LATER THAN the end of the 6th week of classes.
